The Security Rule for HIPAA requires covered entities and their business associates to implement administrative, physical, and technical safeguards to ensure the confidentiality, integrity, and security of protected health information (PHI). These safeguards include implementing risk analysis and management processes; implementing access controls; maintaining audit trails; acknowledging data breaches; and implementing other measures such as training staff, issuing sanctions, and changing passwords.
